To receive our best offer, provide a thorough and accurate description including manufacturer, part/model number and condition. Using the following condition classifications will be helpful:

• OOS = (Out of Service) Functioning properly when removed from service; may have minor issues
• Working = Used, tested and guaranteed working
• Refurbished = Refurbished to like-new condition
• New = Never opened, in original box
• Defective = Known defective

1. Telephone sets - Along with condition and part number, please state color. Note any yellowed plastics or missing accessories.

2. Control unit / KSU - Identify model number of main unit and inventory all removable circuit cards.

3. Circuit cards - Include condition and part number along with version or release.

4. Is software included? - Include part number with version or release. Are transfer rights included?

Other information needed:

• Is the equipment currently in use? If yes, will we be required to send a technician for de-installation and pick-up?
• Where is it located and when will it be available?
• Will you package and prepare equipment for shipment or will we be expected to pick it up?
